Fees at a glance

Mine is a private practice for privately insured patients, people entitled to Beihilfe (the medical allowance for civil servants) and self-paying patients. If you have statutory health insurance, I support you in applying for cost reimbursement. I do not bill your health insurer directly. You receive an invoice from me.

  • Individual therapy, 50 minutes: usually €151.55, exempt from VAT
  • Couples therapy, 75 minutes: expected to be €210
  • Group therapy, 100 minutes: €135 per session
  • ADHD assessment, usually three appointments: about €600, including the written report
  • Second opinion, other assessments and written statements: depending on scope. You receive a written cost estimate in advance, and the fee is payable regardless of the outcome.
  • Coaching, 50 minutes: €170 per session

I bill psychotherapy, but not coaching, under the official German fee schedule for psychotherapists (GOP). The fee for individual therapy may vary depending on the work involved.

How much you get back depends on your insurance and the rules that apply to you. It is best to clarify this before your first appointment.

Private health insurance

Whether and how much your private health insurer reimburses for psychotherapy is set out in your policy. Policies differ widely. Some pay for a fixed number of sessions a year, others only for part of the invoice. Many require written approval after a few sessions. Some older policies only cover psychotherapy provided by doctors. New contracts often have a waiting period of eight months for psychotherapy.

You submit my invoice to your insurer yourself. If your policy only reimburses up to a certain rate of the fee schedule, you may have to pay part of the fee yourself.

If treatment is likely to cost more than €2,000, which is the case from about 14 individual sessions, you can ask your insurer in advance what it will cover. A letter or an email is enough. It must reply within four weeks, or within two weeks in urgent cases. These questions can help:

  • Does my policy cover outpatient psychotherapy with a psychological psychotherapist?
  • How many sessions a year are reimbursed, and what share of the fee?
  • Do I need approval? From which session, and what documents are required?
  • Up to which rate of the fee schedule do you reimburse?
  • Are video sessions reimbursed?
  • Do I need a short report from my doctor (Konsiliarbericht)?
  • Do I have an excess (Selbstbehalt), or will I lose a no-claims refund (Beitragsrückerstattung) if I submit invoices?

In the basic tariff (Basistarif), insurers usually only reimburse psychotherapy with therapists who have a statutory health insurance licence (Kassenzulassung), so not at my practice. The standard tariff (Standardtarif) has lower reimbursement rates. If you are insured under either of these tariffs, please let me know before your first appointment.

Beihilfe

If you are entitled to Beihilfe, the rules for psychotherapy depend on the government you serve: federal civil servants follow the federal Beihilfe rules, Berlin civil servants those of the State of Berlin. If you serve another federal state, its rules apply. The federal and Berlin rules agree on the main points:

  • You do not need to apply for the first trial sessions, usually up to five.
  • Short-term therapy of up to 24 sessions usually does not need approval in advance. In Berlin, the Beihilfe office still asks for an application form for billing, which you and I fill in and sign. Under the federal rules, however, even short-term therapy may need approval in advance. This applies, for example, if your last psychotherapy was less than two years ago or if you are switching to me from another therapist. It is therefore best to ask your Beihilfe office beforehand.
  • For longer-term CBT, your Beihilfe office must give its approval before treatment starts. For this, I write a report that goes to an independent reviewer without your name on it. Usually up to 60 sessions are possible, and 20 more in exceptional cases.
  • By the end of the trial sessions at the latest, Beihilfe requires a medical check to rule out physical causes. Your doctor confirms this in a short report (Konsiliarbericht).

Beihilfe covers a fixed share of the eligible costs: 50, 70 or 80 per cent, depending on your personal situation. The rest is usually covered by your private health insurer. It has its own conditions, so please also ask your insurer whether you need approval. If either of them does not accept the full fee, you pay the difference.

Under the federal and Berlin rules, you can submit invoices up to three years after the invoice date. If you have opted for the flat-rate Beihilfe (pauschale Beihilfe) in Berlin, only your health insurer's rules apply to psychotherapy.

Paying yourself

You can also pay for treatment yourself, however you are insured. You then need no application and no approval, and your insurer or Beihilfe office receives neither an invoice nor a diagnosis from me. We agree together how many sessions make sense, without any limits set by an insurer.

Please note: treatment you pay for yourself is still treatment. If an insurer or a public authority later asks you about psychotherapy, please answer truthfully. This can happen, for example, when you apply for private health insurance or occupational disability insurance (Berufsunfähigkeitsversicherung), or before being appointed as a civil servant (Verbeamtung). Having had psychotherapy does not automatically rule out appointment as a civil servant or an insurance contract. The body concerned decides case by case how it is assessed.

I am bound by professional confidentiality. I only share information with your consent or where the law requires it.

Statutory health insurance: cost reimbursement

My practice does not have a statutory health insurance licence (Kassenzulassung). Even so, if you have statutory health insurance (gesetzliche Krankenversicherung), your health insurer can cover the cost of CBT with me in individual cases. This is possible if treatment is necessary and you cannot find a timely place with a therapist who has a statutory licence (Kassenpraxis).

You apply to your health insurer yourself, before treatment starts. I support you with this, among other things with a checklist and a template for your application. I cannot promise you approval, as your insurer assesses each application individually. Cost reimbursement applies to individual therapy. Statutory health insurers do not cover couples therapy or coaching.

This is usually how it works:

  1. Attend an initial consultation

First attend a psychotherapeutic consultation (psychotherapeutische Sprechstunde) at a Kassenpraxis. The appointment service (Terminservicestelle) on 116117 can also arrange one for you. At the consultation, you receive a form with a treatment recommendation (PTV 11).

  2. Look for a place and keep a record

Ask several Kassenpraxen for a therapy place, and contact each one more than once. Write down every attempt with the date, time, practice, outcome and the waiting time you were given. If you received a code at the consultation, use it to ask the Terminservicestelle as well, and note the result.

  3. Email me

Send me an email saying that you would like to apply for cost reimbursement. You do not need to write anything more. Please do not book an appointment yet. I will send you a certificate confirming an available therapy place and a treatment plan with the expected costs. I send proof of my licence to practise and specialist qualification directly to your insurer.

  4. Submit your application

Send the application with all the documents to your health insurer and keep copies. Some insurers also ask for a short report from a doctor. It is best to check beforehand which documents your insurer requires.

  5. Wait for the decision

Your insurer must decide within three weeks. If it obtains an expert opinion, for example from the Medical Service (Medizinischer Dienst), it has five weeks. If it cannot meet this deadline, it must inform you in good time, in writing or electronically, and give reasons. We only start treatment once your insurer has approved it in writing.

  • Objection if your insurer refuses (German letter with English translation)PDFWord
  • The German application letter to send to your insurerPDFWord
  • The German objection letter to send to your insurerPDFWord

Important: if you have already started therapy or firmly booked appointments before the decision, your insurer can reject the application for that reason alone. You will usually have to pay any costs incurred before its decision yourself.

Insurers often approve a few trial sessions first. You then apply separately for the therapy itself. How much your insurer reimburses per session is stated in its approval. If it reimburses less than my fee, you pay the difference. I will give you the expected costs in writing before we start.

You pay my invoices and submit them to your insurer, which then reimburses you the approved amount.

If your insurer or the Terminservicestelle offers you a place at a Kassenpraxis, please consider the offer seriously. If the timing and the journey are reasonable for you, cost reimbursement is usually no longer an option.

Please send me forms, medical reports and certificates by post rather than by email, as they contain information about your health.

What insurers usually do not pay for

Insurers and Beihilfe only reimburse the treatment of an illness. The following are usually not reimbursed:

  • Coaching, because it is not treatment of an illness.
  • Couples therapy. Statutory health insurers do not cover it, and private insurers and Beihilfe usually do not either.
  • Written statements and certificates, for example for exam adjustments (Nachteilsausgleich).
  • Cancellation fees for appointments you cancel too late or do not attend.

You pay these costs yourself. For written statements and assessments, I will give you the expected costs in writing in advance.

An ADHD assessment, by contrast, clarifies whether an illness is present. Private insurers and Beihilfe may therefore reimburse it, depending on your policy and the rules. For group therapy, they often reimburse only part of the fee. In both cases, please check beforehand.

Frequently asked questions

Will my statutory health insurance cover the costs?

In individual cases, yes. If you cannot find a timely place at a Kassenpraxis, you can apply to your health insurer for cost reimbursement (Kostenerstattung) for individual therapy at my practice. You apply before treatment starts. Your insurer decides whether to approve it. Statutory health insurers do not cover couples therapy or coaching.

Can I book appointments while my insurer reviews the application?

Please do not. If you have already started therapy or firmly booked appointments before the decision, your insurer can reject the application for that reason alone. Please email me first instead of booking via Doctolib or my booking page. As soon as your insurer has approved it in writing, we will arrange the first appointment.

If you need help straight away in the meantime, you will find the numbers under In an acute crisis.

What can I do if my insurer rejects the application?

You can lodge an objection (Widerspruch) within one month, in writing, through your insurer's online form or app, or in person. The exact deadline and the address are given at the end of the decision letter (Bescheid). A simple email is usually not enough. You can send the reasons later, and the objection itself costs you nothing. I cannot give you legal advice.

Are online sessions reimbursed?

Under the federal and Berlin rules, Beihilfe reimburses sessions via a secure video connection in the same way as sessions at the practice. Many private insurers do so too. Please check your policy. Sessions by phone are usually not reimbursed. The invoice states that the session took place by video.

Can I pay first and submit the invoices later?

That is possible, but it does not always work. Many private insurers require approval after a few sessions, and for longer-term therapy Beihilfe must approve it before treatment starts. Without this approval, they often reimburse only future sessions. It is therefore best to clarify this before your first appointment.

With statutory health insurance, reimbursement after the event is usually not possible, because you must apply before treatment starts.

Is therapy for sleep problems reimbursed?

Cognitive behavioural therapy for insomnia (CBT-I) is a form of psychotherapy. If you have a sleep disorder that needs treatment, the same rules apply as for other illnesses. The federal and Berlin Beihilfe rules explicitly list sleep disorders without a physical cause. With private insurance, it depends on your policy. Whether a statutory health insurer approves cost reimbursement is decided case by case.

Can I claim tax relief on the costs?

Possibly. For psychotherapy, the tax office usually requires proof that treatment is necessary, such as a certificate from a public health officer (amtsärztliches Gutachten). This proof must be issued before treatment starts. Please ask a tax adviser about this.
